At Kooth, safety is everything to us. We use the NHS’s Patient Safety Incident Response Framework (PSIRF) to ensure the thousands of people we support every year are safe and supported in the best way possible.
We refer to Patient Safety Incidents as User Safety Incidents (USIs). While some of these incidents will need a full USI investigation, others may call for a different kind of response.
The goal of this process for us is to make the investigation process quicker and more efficient so we can find answers sooner, take prompt action to help prevent harm from happening, and make safety improvements which are regularly reviewed in order to safeguard our users and improve our processes.
The PSIRF is the NHS’s way of learning from incidents that affect patient safety. At its heart, PSIRF is about being open, compassionate and flexible in how we respond when things go wrong. Instead of placing blame, it encourages a deeper understanding of what led to an incident, so that healthcare organisations like Kooth can continue to learn, grow, and make care and support safer for everyone. It provides supportive oversight focused on strengthening response system functioning and improvement, and considered and proportionate responses to patient safety incidents.
